You can remove vqmod xml file from vqmod/xml folder, that will remove the module.

You need to update vqmod file of the module and need to change code for tpl file.

@opencartdeveloper - I've installed this module on version 2.0.3.1. The form is opening a new page and not showing as a popup. Is this how it's supposed to work on version 2.0? I want it to work like the demo site.
