Most problems come from unknown free extensions. If you decide to stay always make a search before installing anything (Who is the dev or company? how long is in opencart? reviews etc...) . Your decision is yours of course.

Replace the tpl file with the original from opencart folder (download again your version). Delete the cache inside vqmod/cache directory. Refresh Modification page in admin Clear your (browser) cache ctrl + F5 at least 3 times Check with another browser Last i would have renamed the vqmod directory ...
