It's included in the Opencart download itself. The file is named .htaccess.txt which obviously needs to be renamed to .htaccess for it to work.

Probably the best approach to handle this would be to go through the steps to upgrade your old store to the same OC version you have installed for the new store. Then migrate the database tables you want to the new install.

DNS propagation can take up to 24-48 hours. So, yes, you should wait longer. Also, hopefully you set the store on the older hosting account to maintenance mode. That way all orders will only come from the new hosting.

Yes, you can do this. Just create a separate database for the forum. It's possible to add tables to an existing db but its good practice to keep a unique db for each different application.
