Adding new fields in Delivery Details section of Checkout

I have a client that would like to add two new fields to delivery details. The two new fields he would like to add are Phone and Email.

Is there a mod that can do this?
Is there a way to add this manually?
When I upgrade, will the upgrade overwrite the changes I make manually?
