If you want something more flexible, you could use Form Builder for this, and replace your Contact Us page with it. That would let you add as many additional fields as you want, and the responses are e-mailed directly to the admin just like the normal contact form.

Though I like the FORM BUILDER, yet it is not free.
I am looking for a free tool at first.
