Does this overwrite core files?

No. It is a bad practice, none of our extensions does that. All changes are applied as ocmod files.

Can the hidden information be picked up by other extensions to work with...like related product extension or other search stuff? Or are they strictly for the administrator to store data to look back on?

The hidden attributes are regular attributes with a special marker. I don't know how other extensions use them, but they should be available. If any 3rd party extension shows hidden attributes unintentionally I can fix it for free.
